The Department of Psychiatry and Behavioral Sciences offers numerous electives which encompass virtually the entire domain of contemporary psychiatry -- and additionally offers students the opportunity to design and negotiate "customized" electives to achieve specific learning objectives. There is also a wide range of research-oriented electives, allowing students the opportunity to explore psychiatry's frontiers, either in the laboratory or in a clinical setting.
The final year of medical school is a unique opportunity in the "long haul" of professional training -- from pre-med through medical school, residency and specialty training. There is time in this fourth year to pursue special interests, of a perhaps more academic, theoretical, or integrative nature. Issues of bioethics, medical judgment, quality of life -- i.e., the psychology of health and health care delivery -- are of interest to some students. There is precious little time to devote to such matters in the whirlwind of professional training, and indeed, there will be even less time available after graduation for such pursuits. Our departments invites you to make the most of this unusual time in your demanding careers.
Students planning to
specialize in Psychiatry are encouraged to arrange electives that will enrich
their knowledge and experience in areas of their special interest. This is an
opportunity to be exposed to areas that may not be covered in-depth during
residency training. Students who consider Psychiatry as a possibility can
fashion an elective that will help them make a decision. Students not intending
to specialize in Psychiatry might consider an elective that will help them make

Students will join the staff of an active acute term general psychiatric unit, providing a rich variety of hands-on clinical experience. Students will evaluate and treat patients in crisis, with supervision by faculty, and become active members of an interdisciplinary team. Treatment modalities employed will include individual, group, family, and milieu therapy, acupuncture, relaxation training, and substance abuse therapies, individual tutorials, case conferences, medical and research and psychopharmacology rounds, and departmental Grand Rounds will enhance students' learning. P508 Latino-American Cross-Cultural Psychiatry Elective for Medical Students
The Jacobi Latino-American Cross-Cultural Units offers an elective in general adult inpatient psychiatry where Latino patients are treated in culturally sensitive and language appropriate manner. Students would be exposed to team approach milieu therapy, including individual psychotherapy, psychopharmacology, family therapy, activity therapy and group therapy. Individual and group supervision will be offered. The elective offers a unique opportunity to differentiate between what is considered pathological in mainstream culture versus what is culturally normative for different Latino groups; learn how to do assessments across the language barrier; how to work with an interpreter; different approaches to engaging members of different cultural groups in treatment.

P516 Phenomenology in Schizophrenia - Bronx Psychiatric Center
The students will earn the PANSS (Poisitive and
Negative Syndrome Scale), and if interested or appropriate other symptom rating
scales (BPRS- Brief Psychiatric Rating Scale, MADRS- Montgomery- Asberg
Depression Rating Scale) for the assessment of schizophrenia. They will
learn how to elicit symptoms and how they cluster ito positive, negative,
cognitive, depression and excitement sub syndromes or "factors". They will
also learn movement rating scales for Soft Neurological Signs, Catatonia,
Cerebellar Signs and Minor Physical Anomolies.
Students will be encouraged to develop their own study to look at aspects of
symptoms and signs: for example, does proverb interpretation improve as other
symptoms do? Does the glabella tap belong in a scale of parkinsonism?
And are we particularly interested in cultural differences in psychopathology
and whether any of these ratings differ according to time of immigration or
country of origin.
Students will be involved in ongoing studies: metabolism in Schizophrenia, some
'add-on' medication studies, surveys of the hospital population of Immigration
and Culture, and Movement Disorders/
Students will earn about risk factors for Schizophrenia, it's geographical and
historical variation, the relationship between psychopathology and neurological
symptoms and wonder about the causes and nature of Schizophrenia.

Addiction Medicine Research: MMC
The Department of Psychiatry &
Behavioral Sciences and Medicine offer this elective work
in Addiction Medicine Research. Several studies are ongoing which include
investigations funded by the Centers for Disease Control and Prevention, the
National Institutes on Drug Abuse and other sources. All are based in the
Division of Substance Abuse, a network of substance abuse treatment programs
with on-site primary medical and psychiatric care. Ongoing investigations include:
A behavioral intervention to reduce HIV risk behaviors and to improve health-care behaviors among HIV infected drug users.
A study of the ways to enhance adherence with HIV medications which compares directly observed administration of antiretroviral medications to an adherence-enhancing counseling intervention.
A national survey of addiction treating physicians regarding their attitudes and knowledge of hepatitis C treatment and drug users.
A study of the pharmacokinetic interaction between methadone and interferon (used in the treatment of hepatitis C infection).
Students will assist in the
evaluation of study participants and in conducting research studies with
research staff. Supervision will be provided by the Principal Investigator

The CPE Program offers a one-month elective in the Acute Management of all psychiatric presentations. Diagnostic interviewing skills with an emphasis on generating broad differential diagnoses will be a major focus of this elective.

P524 Psychiatry Research- AECOM
EThe primary aim of the Lachman lab is to
understand the genetic basis of schizophrenia and bipolar disorder, two of the
most disabling conditions on the world. Over the past decade, a number of
genes involved in these disorders have been identified. However, the few
exceptions, the causative alleles have not been found. One reason could be
that genetic variants occurring in regulatory domains that are relatively far
removed from the transcription start site are involved. One of our areas
of interest, therefore, is to identify distal regulatory domains and determine
if functional genetic variants in those regions are associated with
schizophrenia and biploar disorder. A ChIp on chop approach is being used
to characterize functional domains. As a result of our studies, several
novel functional domains and regulatory pathways in candidate genes have been
identified, which are currently being analyzed in the lab. No previous
laboratory experience in needed; the techniques used in our studies are
relatively easy to learn.

P526 Research in Behavioral Genetics
Our major interest is identifying genes involved in psychiatric disorders including bipolar disorder, schizophrenia, violent behavior and substance abuse.
1.Candidate gene analysis in bipolar disorder: We have recently identified a candidate gene for a severe form of bipolar disorder known as ultra rapid cycling. Additional patients are being recruited into the study in order to replicate our original findings. In addition, we have used genetic linkage analysis to identify a locus on chromosome 22 that may contain a
The lab has been
accumulating evidence that the genetic variant that causes low COMT activity is
involved in substance abuse vulnerability and increased violent behavior in
chronic schizophrenics. These are ongoing studies that we are attempting to
replicate.
Although previous laboratory experience would be a plus, students without
extensive research experience are welcome. We have training many inexperienced
students in basic recombinant
DNA techniques. The principal investigator does not believe in the "sink or
swim" philosophy of teaching. Laboratory techniques will be learned by
direct instruction and students will work independently only when they feel
comfortable with the material.

There is considerable individual variation in the degree of nicotine addiction among smokers. Twin and adoption studies have provided evidence for the hypothesis that this variation in part depends on differences in their genetic composition. Moreover, individuals at increased risk for nicotine addiction exhibit a host of co-morbid traits, including novelty seeking, anxiety, impulsiveness, aggression and depression. (Hiroir and Agatsuma, 2004) It remains unclear, however, exactly what genes contribute to the variation to nicotine addiction and novelty seeking. Genetically engineering mice that lack the fosB gene show heightened responsiveness to nicotine's reinforcing effects and novel stimuli (Hiroi et al., 1997; Zhu et al., 2001).
Our research uses genetically engineered mice to ascertain the role of foxB in nicotine addiction and it's co-morbid traits. Students will have the opportunity to learn behavioral, anatomical and biochemical techniques.

The
Departments of Psychiatry & Behavioral Sciences and Medicine offer this
elective in substance abuse to fourth year students. Students will
participate in clinical care in both inpatient and outpatient settings and will
be responsible for working up and presenting patients to attending physicians.
Inpatient experience will be with the Substance Abuse Consultation and Referral
Service (SACRS) at Montefiore Medical Center, which consults on hospitalized
patients with substance abuse disorders and provides guidance on the management
of acute issues, such as, withdrawal. Outpatient experiences will include
observation and participation in the following settings:

P701 Adult Psychiatric Day
Program- LIJ/Hillside Hospital
This elective allows a student to become
acquainted with the structure and function of a day treatment program and with
the clinical management issues involved for patients in treatment with this
modality.
Day treatment is a unique and emerging field of psychiatry. The Hillside
Day Program offers an opportunity for an extended five-day-a-week treatment to
seriously ill or disabled patients not requiring 24-hour hospitalization (with
its attendant high cost and consequent pressures for rapid discharge). In
this setting, intensive interventions may be undertaken with close monitoring of
the patients condition and adjustment of treatment in a more realistic time
frame in time frame in terms of what we currently know about the course of
psychiatric disorders and the recovery process. Moreover, treatment in
this setting is measured against “real life”.
Patients spend evenings and weekends at home, have
an opportunity for leisure and social activities and can plan for serious
vocational preparation and rehabilitation within the context of the program.
The Hillside Adult Day Program is divided into distinct components; a partial
hospitalization program, a continuing treatment program with rehabilitation and
relapse prevention tracks. Patients are assigned to these programs on the
basis of their treatment progresses.
A student participating in this elective will have
an opportunity to become familiar with the various components of day treatment
and develop expertise in modalities, which are likely to become increasingly
important and more highly utilized in Psychiatry. This elective is offered

P702 Evaluation Center-
LIJ/Hillside Hospital
The Evaluation Center functions as a combination
psychiatric crisis intervention service and brief psychotherapy service.
As a psychiatric crisis service, the Evaluation Center provides professional
consultation to the LIJ Emergency Room during the day. The Center also
evaluates patients who come in without an appointment.
Different approaches to care may include
appointments/referrals to private clinicians, clinics or day programs and
psychiatric hospitalization. The Center also offers brief psychotherapy to
patients who could benefit from this approach. This brief therapy is
flexibly defined and may call for such activities as clarifying aspects of the
patient’s life which are producing distress, environmental manipulation (e.g.,
patient may need referral to social service agencies), family interventions,
medication and support. An ambivalent patient may be seen briefly with the
hope that he can be motivated to accept further needed treatment.
During the rotation in the Evaluation Center, the
student will have an opportunity to experience first-hand the assessment of a
patient in crisis and receive direct supervision by the Attending Psychiatrist.
BY successfully completing a rotation in the Evaluation Center, the student
should be competent in the rapid assessment of psychiatric emergency situations.
Depending on the student’s interests. There may be time to participate in the
intake/treatment through the Ambulatory Care Center. This outpatient
facility is divided into a Diagnostic and Assessment Unit and six specialty
clinics; Affective (mood) Disorders, Phobia-Anxiety Disorders, Geropsychiatry
and Human Sexuality. This elective is offered is offered throughout the
year.

P703 Medical Clinic- LIJ/Hillside
Hospital
With the ever-increasing recognition of the interface that can occur between medical and psychiatric illnesses, a month-long elective based in the Medical Clinic at Hillside Hospital is being offered. The student will participate in the following activities:
1. Half-day sessions in Neurology
2. Primary medical care of both hospitalized and ambulatory patients with psychiatric disorders.
3. Medical Consultation directed at the interface of Medicine and Psychiatry (e.g., medical causes of behavioral disorders).
4.
Clinical conferences involving Psychiatry, Neurology
and Medicine.
During a rotation in the Medical Clinic, the student will be taught how to differentiate between psychiatric and medical conditions; proper diagnostic evaluation procedures as well as knowledge of the drug interactions between psychotropics and other therapies (e.g., ECT), medical contradictions to their use and appropriate ways in which to follow their use in medically complicated patients.

P704 Geriatric Psychiatry-
LIJ/Hillside Hospital
The broad-based goal of this program is in-depth exposure to the rapidly developing field of Geriatric Psychiatry. Medical students can participate in a wide range of clinical activities at diverse sites, including a 20-bed geriatric psychiatry inpatient unit, a comprehensive geropsychiatry outpatient clinic, a geriatric psychiatry day hospital, a community-based program, and home care programs and a long term care facility. They will have exposure to ongoing research initiatives in late-life depression and dementia, Tardive Dyskinesia, brain imaging and psychiatric health care delivery in nursing homes. Students will participate in structured educational activities that include clinical case conferences, family conferences, geropsychiatry Journal Club and didactic curriculum.

P705 Inpatient Psychiatry-
LIJ/Hillside Hospital
The medical student will be on one of three
inpatient service units and will have primary responsibility for the care of the
assigned patients. This includes the history, physical and psychiatric
examinations, laboratory examinations, psychiatric diagnostic assessment and
management of newly admitted patients.
The student will receive individual supervision
from an Attending physician and depending on the unit, work alongside PGY1 or II
Residents. Part of the experience will involve participation in didactic
seminars, educational conferences in psychopharmacology and movement disorders,
as well as, clinical cases involving Neurology, Medicine and Psychiatry.
Depending on the student’s interest, this rotation would also include time spent
in either the Evaluation Center or Outpatient Substance Abuse Clinics.

(1) Longitudinal studies of newly admitted schizophrenic
patients including MRI and
cognitive functioning.
(2) Clozapine treatments of patients shown to be refractory
to standard narcoleptics or with
severe Tardive Dyskinesia.
(3) A survey of axis II diagnosis in outpatients with
alcohol/dependence.
(4) Neuropsychiatric Rehabilitation.
(5) Applications of MRI brain-imaging technique and
neuropsychology to the study of brain
abnormalities in schizophrenia,
Alzheimer's disease and epilepsy.
(6) Research in mental retardation.
(7) Unilateral ElectroConvulsive Therapy vs. Bilateral ECT.
Other projects are planned. Students will participate in weekly research
meetings, receive individual supervision from a senior attending and if desired,
get assistance in writing a paper.
This is a unique opportunity to learn what clinical psychiatric research is like
from the "nuts and bolts" view of an insider.

The purpose of this
elective is to acquaint the medical student with what consultation-liasion
psychiatry is all about. Students
will see patients who are having emotional reactions to psychical illness, such
as, Cancer and Heart Disease, and learn how to treat them. They will also learn
the impact of psychiatric comorbidity on general health care, as well as, how
the emotion effect specific medical disorders, such as, granulomatous bowel
disease and heart disease. Students will be supervised by the Fellows and staff
of the C-L service and work with them on actual cases. In addition, the
students will participate in Neurology Teaching Rounds, Renal Dialysis Rounds,
Med-Psych Teaching Rounds on Medicine and a weekly staff-wide C-L Clinical Case
Conference. They will also gain experience in the use of biofeedback in
psychosomatic disorders by observing the actual treatment of patients as well as
being given brief didactic exposure to the topic. Other specialized experiences
are available on request.

In recognition of the fact that the students

menu if different electives that is offered at Hillside (Adult Day Hospital,

P709 Elective Substance
Abuse/Community Psychiatry- LIJ/Hillside Hospital
This elective exposes students to
comprehensive substance abuse treatment and to patients in varying degrees of
recovery from this type of psychiatric illness. Students may choose to join
clinical teams from a variety of community based and hospital programs. With
nine separate sites to select from and the opportunity to work at different
sites simultaneously, a meaningful experience can be fashioned to suite almost
any interest and educational need.
Students may elect to work with patients of different ages, ranging from adolescents to elderly adults. There is also the opportunity to gain experience with different treatment modalities, such as, Methadone maintenance, medically monitored detoxification and group psychotherapy. Those students with research interests are welcome to participate in ongoing protocols. In all electives, the student is given on-site supervision.

Neurology in a Psychiatric Patient Population- LIJ/Hillside Hospital
During this elective, students have the opportunity to work in the realm that is the interface of psychiatry and neurology. Individual supervision with an attending neurologist who specializes in this patient population forms the core of this rotation. Clinical work will encompass neurologic conditions in both inpatient and outpatient psychiatric populations. Additional clinic exposure and specialty evaluations (i.e., movement disorder, seizure, etc.) enhance the experience.

Inpatient Psychiatry- Beth Israel Medical Center
Each student will be responsible for the care
of approximately four inpatients with resident and faculty supervision. Student
will follow their patients from initial evaluation to discharge planning and
will learn the factors that lead to a decision to admit, gain skills in
diagnosis and the use of medications and understand the relationship among
biological, psychological and social factors in psychiatric disorders. Students
will be integrated into an interdisciplinary team and will learn how to act as a
team leader for their patients. The service will try to accommodate a student’s
needs and interests.

Students will be responsible for the evaluation and care of the patients, substance abuse and other psychiatric disorders. Students will be assigned primarily to the dual diagnosis inpatient unit. The student will become familiar with the evaluation and treatment of the complex Biopsychosocial problems associated with this population as well as with the outpatient treatment resources.

understanding the interplay of biological, psychological and social factors in
the occurrence and course of psychiatric disorders. Students will be assigned

P804 Subinternship in
Geriatric Psychiatry- Beth Israel Medical Center
Students will be responsible for the care of
aged psychiatric inpatients with a wide range of psychopathological disorders.
In addition, the student will have outpatient experience in the comprehensive
evaluation and treatment of patients 65 years and older. The student will
become familiar with the complexity and special problems of the aged psychiatric
patient using the Biopsychosocial research. The student will coordinate the
psychiatric, medical, neurological and social evaluation of elderly patients.
There will be supervision in individual and family therapy, as well as exposure
to working with outside agencies. Opportunities for geriatric consultation can
also be arranged. Readings will be provided to further assist the student in
understanding the process of normal aging and the multitude of pathologic
processes that may rise.
