Students will observed and participate in the
clinical and didactic activities of the Physical Medicine and
Rehabilitation (PMR) training program. All students will develop an
appreciation of the PMR practice methodology, including a team approach to
the treatment of disordered function.
The elective provides an overview of the clinical services offered by PMR.
Students will observed the planning and delivery of both physical medicine
and rehabilitation care including: acute and subacute general
rehabilitation, Musculoskeletal medicine, electrodiagnosis, cardiac
rehabilitation, physiatric procedures (including joint and soft tissue
injections, botulinum toxin injections for spasticity management, spray
and stretch, unna boot application and peripheral nerve blocks.),
prosthetics, orthodics, complementary and alternative medicine techniques,
wound and foot care and pain management.
The elective is located at Montefiore and Einstein hospitals and the
Montefiore Medical Park as well as Jacobi Medical Center and Beth Abraham
Hospital. Students evaluate and treat inpatients and outpatients in
tandem with attending faculty. They also become familiar with the
treatment modalities employed by physical and occupational therapy and
speech/language pathologists.
Students participate in daily work and teaching rounds, team conferences
and outpatient clinics. They are responsible for participation in
the didactic program, including journal club and other teaching conference
presentations. Research opportunities are available upon request.

Students will participate
in a private medical practice devoted exclusively to the treatment of
diabetes and its complications. Patients of all ages will be seen. Participants
will be trained in the most advanced methods for outpatient blood sugar
normalization and weight control.
Topics to be covered
include: patient training, intensive physical examination geared toward
tracking diabetic complications, routine and research level laboratory
studies, special diagnostic equipment, diet, self monitoring of blood
glucose, exercise, intensive insulin therapy, selection and use of oral
hypoglycemic agents, etc. Diagnosis and treatment of the following diabetic
complications will be covered in depth: gastrointestinal motility disorders,
autonomic neuropathies, peripheral vascular disease, hyperlipidemias,
foot ulcers, fungal infections, hypertension retinopathy, cataracts, erectile
impotence, etc.

Students must provide their own transportation to Mamaroneck, New York
(15 miles north of AECOM). Buses run from the Bronx and trains from
Fordham Station. First day of elective, meet at 9:00 a.m. - 1160 Greacen
Point Road -Mamaroneck, New York (maps and transportation details are
available from preceptor).
The student will be
introduced to the specialty of Physical Medicine and Rehabilitation in
a variety of subacute and long-term care settings. The student will become
familiar with the principles of both subacute restorative rehabilitation
for patients of all ages, and geriatric long term rehabilitation. The
student will also be introduced to operations in a skilled nursing facility,
an adult day health program, and CCM (a comprehensive managed care program)
designed to permit the frail elderly and disabled who qualify for nursing
home care, to remain functioning in their community. The student will
observe and participate in the interdisciplinary rehabilitation team process,
and become familiar with physical, occupational, speech and hearing, music
and recreation therapies. The roles of the social worker, public health
nurse and physician's assistant will be addressed. The student will attend
the rehabilitation clinic, prosthetic and orthodic clinic, wheelchair
clinic and the electrodiagnostic laboratory.
In addition, the
student will be familiarized with the principles of functional anatomy,
neuromuscular dysfunction, and the application of the computer in cognitive
perceptual evaluating training, biofeedback in muscle re-education, and
augmentative communication. The student will have the opportunity to participate
in a home visit with therapists to plan for independent and safe community
living.

Students will become familiar with all aspects of testing including nerve conduction studies, electromyography and evoked potential studies.

J801 Prosthetic-Ortho Restoration of Multi-Handicapped Patients- Beth Israel Medical Center
Students will participate in the functional evaluation of multihandicapped patients who require either an artificial limb or bracing or splinting. Educationally, exposure will also include biomechanical assessment and management systems utilized with these individuals.

Cardiac Rehabilitation is an well-accepted treatment modality for the patient recovering from an acute myocardial infarction, angina, or coronary bypass surgery. This program includes monitored medically supervised exercise sessions, psychological counseling and dietary instruction. At the end of the rotation, students will learn how to evaluate the patient and to develop an appropriate exercise program. Additionally, the student will become acquainted with the use of the arm ergometer in the functional evaluation of the patient with cardiac disease and lower extremity disability. Additionally the student will become acquainted with the comprehensive rehabilitation management of the patient recovering from bypass surgery and/or valve replacement.

Student will become familiar with the diagnosis and management of both surgical and non-surgical Musculoskeletal entities. State of the art applications of research based training paradigms will be emphasized.
